This agreement outlines the terms for a one-time consultation session with Shelly Sowell, owner of Shelly Sowell Mindful Education LLC.

1. Nature of the Consultation

The consultation is a single coaching session designed to:

• Clarify current goals or challenges

• Explore alignment and fit for potential coaching services

• Identify possible next steps

This consultation is coaching in nature and does not constitute therapy, counseling, or a therapeutic relationship.

Participation in this consultation does not establish an ongoing coaching agreement unless both parties mutually agree to enter into one.

2. Fee & Payment

The consultation fee is due at the time of booking. All consultation fees are non-refundable, however, you may reschedule up to two (2) times. If you choose not to pursue further services, no additional obligation is required.

3. Rescheduling Policy

You may reschedule your consultation up to two (2) times with at least 24 hours’ notice.

After three reschedules, or in the case of missed appointments without notice, the consultation fee is forfeited. This flexibility is offered out of respect for others and how life can contain unexpected challenging circumstances.  However, both parties agree to respect each other’s time as valuable and only reschedule when it is a necessary circumstance, rather than personal preference.

4. Confidentiality & Professional Responsibilities

I, Shelly Sowell, owner of Shelly Sowell Mindful Education, LLC, am a Licensed Professional Clinical Counselor in the State of Kentucky. I am also an experienced educator, consultant, and coach. This agreement is for the latter professional roles. 

This consultation is not a form of therapy and does not establish a therapeutic relationship. However, I remain ethically and legally obligated to uphold professional responsibilities in accordance with my license, including mandated reporting laws.

Confidentiality may be limited in circumstances including, but not limited to:

• Risk of harm to yourself or others

• Suspected abuse or neglect of a child, elder, or vulnerable adult

• Court order or other legal requirement

Coaching services do not carry the same legal privilege protections as healthcare and psychotherapy services. While I take care to protect your privacy, coaching communications may not be afforded the same statutory protections as psychotherapy. If you have questions about confidentiality, we can discuss them before or during the consultation.

5. No Emergency Services

This consultation is not appropriate for crisis or emergency support. If you are experiencing psychological distress or an emergency, please contact:

• 911 (U.S.)

• Your local emergency services

• Or 988 (Suicide & Crisis Lifeline in the U.S.)
  Email or scheduled consultation sessions do not substitute for immediate care.

6. Technology & Location

Sessions are conducted via Zoom unless otherwise arranged.

Clients are responsible for ensuring a private, stable internet connection and an appropriate environment for the session. I build in 10 minutes in my schedule for any tech issues that may arise. 

7. Scope of Services

The consultation is a reflective, growth-oriented conversation. Outcomes are not guaranteed. The purpose is exploration, clarity, and discernment regarding possible future work.

8. Agreement

By booking and attending this consultation, you acknowledge that:

• You understand this is a coaching consultation, not therapy.

• The fee is non-refundable.

• You agree to the rescheduling policy.

• You understand this session does not establish an ongoing professional relationship.
  

End of Agreement.  Agreement is signed by checking the box on the Acuity form. Or if you receive this independently through Shelly Sowell and an E-signature service.
